Admission, Discharge, Retention Decision Protocol
Policy  
Bayshore Manor assesses individuals for appropriateness for a basic level assisted living facility in accordance with State Statutes and Rules and Regulations set by the Florida Department of Elder Affairs.  Such functional assessment includes observation, conversations, consultations, history and evaluation of the Health Assessment.


Purpose 

To establish guidelines governing the admission, retention and discharge criteria for Bayshore Manor.


Procedure       

A.      All prospective applicants complete an interview with the Sr. Administrator of Bayshore Manor during which time, the ability of Bayshore Manor to fill the needs of the applicant will be evaluated by the Sr. Administrator.

B.      If the Sr. Administrator believes the applicant could be served, the applicant then makes application and determines financial eligibility through the Monroe County Social Services Department.

C.      Upon completion of application, if a bed is available, the prospective resident will be referred to his or her healthcare practitioner for completion of the required health assessment.  If a bed is not available, the prospective residents’ name will be added to the waiting list.  In this case, the health assessment will be required when the person’s name comes up on the waiting list.

D.      If the person is judged appropriate for Bayshore Manor, a move-in date is set.

E.      General admission criteria follow:

·       Must be at least 60 years of age
·       Must be a resident of Monroe County
·       Must be capable of personal care with supervision or assistance
·       Must not require more than supervision with medications
·       Must be able to participate in activities at some level.
·       Must not be incontinent of urine or feces
·       Must not exhibit violent behavior
·       Must be free from signs and symptoms of communicable disease which could be transmitted to other residents or staff.  This includes a tuberculin skin test or recent chest x-ray to rule out tuberculosis.
·       Must not require skilled nursing care, except that which could be provided by home-based nursing care.
·       Must not be judged legally incompetent, or if so judged, must have a legal guardian willing to be the responsible party.
·       Must be able to evacuate the facility in case of fire.

F.      Retention criteria include all of the above (E) and a current (within 1 year) health assessment which states the resident is appropriate for a basic assisted living facility.  Should the Sr. Administrator determine that the resident no longer meets criteria for continued stay, the resident will be referred to the private physician for an assessment and placement guidance.

G.      Discharge will be done under the following situations:

·       By the resident or responsible party at any time
·       If the resident vacates the facility
·       Upon certification by a physician that the resident no longer meets criteria for basic level assisted living
·       Upon determination that the resident exhibits such chronic behavior as to interfere with the care and comfort of other residents
·       Upon non-payment of amounts due
·       Upon closure of Bayshore Manor
·       Forty-five days notice of need to vacate the premises will be given, except where the resident requires a higher level of care than that for which the facility is licensed.
